New Web User Setup

 

Please read these instructions for setting up your new login information. After reading through these steps, select the link at the end of this page to begin the user setup process.

Participant Login

  1. Enter your initial User ID (your SSN without dashes)  
  2. Enter your initial Password (your date of birth, entered as mmddyyyy)
  3. Click Login

 

  

Create New User Options

  1. Enter a User Name in the first box (between 8 - 20 characters, no dashes)
  2. Enter a Password in the second box  
  3. Re-enter your new Password (to ensure it matches)
  4. Create a challenge question and answer (e.g., What is your favorite football team, etc)
  5. After clicking ‘Next’ you will be taken to the appropriate website

* Please note:  If you are in more than one Plan you will be asked to select the Plan before being directed to the site.

Participant Login Link

Once you have read through the steps, select the following link to access the Participant Login page where you will enter your initial User ID and Password.
